Pre-Registration for OB Admission



Thank you for choosing ST. FRANCIS eastside for your upcoming delivery! Pre-registering for your hospital stay allows your admission to happen quickly and smoothly. We strongly recommend that you pre-register by your fourth month of pregnancy so that all paperwork is completed well in advance of your delivery. Follow these three easy steps to pre-registering at St. Francis:

Please confirm that you have signed and made copies of the following:
  • The five forms indicated in the OB Registration Packet
  • Copies of your Insurance Cards
  • Pre-Registration Form  (only if you did not complete online)
There are two easy ways to return your completed forms:
  • Mail in an envelope to:

  • Drop off in person to:
    Registration Desk, Entrance B
    ST. FRANCIS eastside

Registration Confirmation

 
After you have returned the required documents, you will receive a registration confirmation in the mail. Confirmations are usually sent within two weeks of the receipt of the signed forms. If any information is missing from your forms, we will call you at the number you provided on the OB registration from.
 

When You Arrive at the Hospital

 
If you receive a registration confirmation, we have all we need to admit you as a patient. When you arrive at ST. FRANCIS eastside for delivery, you will only need to provide a photo ID, your insurance card and any additional forms your physician has asked you to bring with you. After confirming your name, due date and insurance, you will go directly to the fourth floor with an escort.
